Patient Referrals:

Patients may be referred to the Foothills Regional Pain Center by their primary or consulting physicians by contacting our office directly, or by using our physician referral form. A copy of this form may be downloaded as an Adobe Acrobat document at the link provided below and may be faxed or mailed to our office. If desired, the form may be completed and given to the patient to bring to the office at the time of the referral visit.

Offsite/Internet Evaluation Service:

As a courtesy to potential laser discectomy patients traveling from a distance, Foothills Regional Pain Center offers an Offsite Evaluation service. One of our physicians will speak with the patient by phone and review MRI films to determine if the patient is not a candidate for laser discectomy. Knowing from the outset that a patient is not a candidate for laser discectomy will prevent unnecessary travel and lodging expenses to come to our Upstate SC offices for in-office evaluation. The patient will receive a letter discussing the history, MRI findings, and pain symptoms which may respond to laser discectomy, or why they would not. The cost for this service is $250.00, payable when MRI films are received. Alternatively, standard in-office evaluation is available for all patients who desire to be seen and examined.
